The Long-Term Health Effects of Shift Work: What the Research Really Says

Cardiovascular disease, diabetes, cancer risks. Here is the evidence without the fear-mongering.

The WHO/IARC Classification

In 2007, the International Agency for Research on Cancer (IARC), part of the World Health Organisation, classified night shift work as "probably carcinogenic to humans" (Group 2A). This placed shift work in the same category as exposure to diesel engine exhaust and ultraviolet radiation.

The classification was based on sufficient evidence from animal studies and limited evidence from human studies. More recent meta-analyses have strengthened the human evidence, particularly for breast and prostate cancers.

Cardiovascular Disease

A 2018 meta-analysis in the BMJ covering over 2 million participants found that shift work was associated with:

  • 9 percent increased risk of coronary events
  • 24 percent increased risk of myocardial infarction
  • 5 percent increased risk of stroke
  • The risk increased with years of exposure. Workers with more than 10 years of shift work experience had higher risks than those with less exposure.

    Metabolic Effects

    Shift workers are 1.3 to 1.5 times more likely to develop type 2 diabetes compared to day workers, according to a 2015 study in Diabetes Care. The mechanism is circadian disruption affecting glucose metabolism and insulin sensitivity. When you eat at times your body expects to be sleeping, your metabolic response is impaired.

    Putting It In Perspective

    These relative risk increases sound alarming. In absolute terms, they translate to small but meaningful increases in individual risk. For context, smoking increases lung cancer risk by roughly 20-30 times. Shift work's cancer risk increase is approximately 1.3-1.5 times.

    The goal is not to panic. It is to make informed decisions with the best available data.

    What Reduces the Risk

    **Pattern design matters.** Rotating shifts (rapidly changing between day and night) are associated with worse health outcomes than consistent night work. Forward-rotating schedules (days to evenings to nights) are better than backward-rotating schedules.

    **Recovery matters.** Longer recovery periods between shift blocks reduce the cumulative biological impact. The research consistently shows that the risk profile improves with adequate recovery time.

    **Lifestyle matters more for shift workers.** Because shift work adds baseline metabolic and cardiovascular risk, the protective effects of exercise, good nutrition, and non-smoking are amplified. These are not optional extras. They are essential countermeasures.
